2019 INDOOR REGISTRATION INFORMATION

Time To Move Indoors for the Winter!!

ALL ATHLETES WILL NEED: SHIN GUARDS, MOUTHPIECE AND A STICK.

EVERYONE WILL NEED A THICK SOCK TO PLACE OVER THE END OF THEIR STICK TO PROTECT THE GYM FLOOR. (EVEN THOSE ATHLETES WITH INDOOR STICKS)

Dates: (Tentative) February 16, 23 March 2, 9, 23, 30 (April 6- Make-up date only, if needed)

HIGH SCHOOL LEAGUE DATES: Same as above
All high school teams will be handled by the high school representative and players should BE SURE to note on the form which high school they will be playing with. All questions regarding individual teams should be directed to that representative.

League Cost: $85**
**Add additional fee of $10 if registration is received after January 10th.
Registration deadline: January 15, 2019 (Registrations still accepted after this date. Just add late fee.)
Return Check Fee: $35.00

Schedules will be completed once sign-ups have closed. Skill groups/games will begin February 13 and will run for 6 weeks on Saturday mornings only. There will be a short practice/skill session prior to games. There will no be permanent teams. Each week, players will be divided into teams to play.

Please note: After 5/10/21, your requested tank top size is not guaranteed due to late sign up. Extra tanks are ordered at registration deadline. If you would like to receive your child's exact size you may incur additional printing fees. In most cases, this all works out. However, I have to share this disclaimer.
Photo Waiver

I understand and will allow photos and/or videos of me and/or my child to be taken while at this activity to be used in any Girls Field Hockey, Ltd. publications/social media. I also understand that publication of these photographs may be accomplished electronically via the Internet/World Wide Web and that after publication, Girls Field Hockey, Ltd. will be unable to prevent persons from gaining access to the Internet/World Wide Web, copying my photographs and video therefrom, and subsequently using, altering, or republishing it without my consent. I waive any claim for damages against Girls Field Hockey, Ltd. from unconsented use, alteration, or republication of my photographs and video by third parties accessing the Internet/World Wide Web.

Release of Liability/ Injury Waiver*

As a parent of a child participating in Girls Field Hockey, Ltd. organization, I retain medical expense responsibility and do hereby release, acquit, hold harmless and forever discharge GFH, its agents, agencies, servants and employees, and all persons, natural and corporate, in privity with them or any of them from any and all claims or causes of action of any kind whatsoever, including but not limited to actions, suits, and/or claims form any bodily injuries, deaths, or property damage which may be sustained by , my child(ren) while participating in any activity, including travel to and from such activities, not resulting from the intentional tortuous act or acts of any agent, servant, or employee of GFH. It is acknowledged that the decision to engage in the above activities is entered into freely, and that GFH, its agents, agencies, servants and employees have not influences the decisions to engage in such activities.

ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT In consideration of being allowed to participate on behalf of Girls Field Hockey, Ltd. athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that: Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (insert name of sports organization) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
